Subrecipient Commitment Form

This form is required to be completed and signed by the authorized institutional official of an organization participating on a Cornell-led project as a subrecipient. Organizations who are Federal Demonstration Partnership (FDP) Expanded Clearinghouse participants are required to complete the first section (page 1) and have the form signed by an authorized representative on the top of page 2. (See FDP Expanded Clearinghouse Program for additional information.) All non-FDP participants must complete all sections of the form and have it signed by an authorized representative on the top of page 2.
